Facility Evaluation Report

On 12/22/25 at 3:55PM, Licensing Program Analyst (LPA) D Panlilio conducted a case management visit and met with administrator (ADM). LPA explained the purpose of the visit with ADM.

At 4PM, LPA toured the facility including but not limited to four (4) bedrooms, two (2) full bathrooms, dining room, living room, laundry room and kitchen. LPA observed the facility clean, odor free and in good repair.

LPA observed no clients at the home. ADM stated the remaining two clients moved out of the facility on 12/20/25 and safely relocated to another Adult Residential facility in Oakley CA. ADM decided to close the facility due to retirement.

At 4:10PM, ADM surrendered the facility's State License to LPA for facility closure.

No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
