Facility Evaluation Report
On 08/14/25 at 1PM, Licensing Program Analyst (LPA) D Panlilio conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 08/08/25. LPA met with Licensee/Administrator (ADM) and explained the purpose of the visit.
LPA discussed self-reported incident dated 08/08/25 with ADM which showed (S1) physically abused client (C1) several times as witnessed by another staff (S2). S1 made serious physical and death threats to S2 and his family. ADM talked to S1 who voluntarily left the facility on 08/06/25.
ADM called local police and filed a report (Case# 25-6A3A). LPA observed S1 was not present at the facility during visit. ADM confirmed with LPA that S1 voluntarily resigned and left the facility on 08/06/25.
No deficiencies cited. Exit interview conducted and a copy of this report provided.
