Facility Evaluation Report

On 1/6/2026 at 3:00pm, Licensing Program Analyst (LPA) L. Hall, conducted an unannounced annual required inspection. LPA met with Oyesiji Oyewusi, Direct Care Support explained the purpose of the visit. Administrator, Enibokun Oyewusi, arrived at 3:38pm. Administrator holds certificate #7028068735 expires 7/1/2026. The facility’s fire clearance was approved for six (6) ambulatory clients.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of three (3) bedrooms and one (1) bathroom. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 69 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 111.4 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents.

Smoke detectors/carbon monoxide were in operating condition during visit. The emergency disaster plan was last reviewed 1/02/2025. Fire extinguisher was last purchased on 12/11/2025. Fire drill last conducted 11/18/2025. First aid kit was observed to be complete.
