Facility Evaluation Report
On 5/20/2026 at 9:45am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Brandon Lopez, Direct Care Support at 10:10am. LPA spoke with Administrator, Emmanuel Oyeleke, via telephone, and explained the purpose of the visit. The administrator currently holds a certificate (#7028400735) that expires on 09/13/2026. The facility’s fire clearance was approved for four (4) non-ambulatory.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of three (3) bedrooms and two (2) bathrooms. All indoor and outdoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 110.2 degrees Fahrenheit. All toilets, hand washing, and bathing are in operating condition. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.
Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher was last services on 10/17/2024. Emergency disaster plan last reviewed 5/7/2025. First aid kit was observed to be complete.
