Facility Evaluation Report
On 3/24/2026, at 11:15am, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit regarding an incident report that was submitted on 3/16/2026. LPA met with Jennifer Cosme Romero, Direct Care Support. Dayanis Matos Tur, House Manager, arrived at 11:43am, and LPA explained the reason for the visit.
The incident occurred on 3/15/2026, where C1 was not feeling well. The incident report stated that staff checked C1's glucose. S1 stated that she and the Administrator are licensed professionals and has trained the staff to conduct the glucose testing. LPA observed S1 certification as a licensed professional. LPA also reviewed S2, S3, and S4 files and observed they have medication training. LPA advised S1 that the training for glucose must be documented in each file.
