Facility Evaluation Report

On 8/27/2025, at 5:20pm, Licensing Program Analyst (LPA) L. Hall conducted a case management visit. LPA met with Noah Ramirez, Direct Support Staff and explained the purpose of the visit. Administrator, Sarena Williams, arrived at 5:52pm.

Upon arrival, LPA observed a total of four (4) clients and one (1) staff. Three (3) clients were sitting in common area and one (1) client was in his bedroom. Staff was making dinner. Clients in care appear to be safe and there are no imminent health/safety concerns on today's date. Facility is noted to be clean and in good repair on today's date.

At 5:25pm, LPA observed there was only one (1) staff present with four (4) clients.

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC809D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
