Facility Evaluation Report
On 7/28/2026 at 1:35pm, Licensing Program Analysts (LPAs) L. Hall and C. Fowler conducted an unannounced Case Management visit nonpayment of civil penalties. LPA met with Ricky Hazan, Direct Care Support. LPA spoke with Tracy Mitchell, Direct Care Support, via telephone and explained the purpose of the visit.
LPAs conducted visit to deliver the invoice for civil penalties that were issued on 10/08/2025 in the amount of $250.00.
Upon arrival LPAs observed there were four (4) clients and only one staff. One (1) client was in the garage, two (2) clients were sitting n the common area, and one (1) client was in his room. Per regulation 85065.5 there should be one direct care staff to three such clients. S3 arrived at 1:58pm.
*A civil penalty of $250.00 will be issued on today's date for a repeat violation of regulation 85065.5(a)(1)*
