Facility Evaluation Report
On 6/3/2025 at 2:10pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Rosa Escorza, Caregiver . Administrator, Rafael Rubio, arrived at 2:25pm, and LPA explained the purpose of the visit. The administrator currently holds a certificate (#7026021735) that expires on 10/25/2026. The facility’s fire clearance was approved for six (6) ambulatory clients.
LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, and back yard. The facility consists of three (3) bedrooms and two (2 ) bathrooms. All indoor and outdoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Clients rooms have proper bedding and linens for the residents to use. Hot water temperature in the shared clients’ bathroom was measured at 110.5 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Bathrooms were equipped with non skid mats. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.
