Facility Evaluation Report
On 04/07/26 at 2PM, Licensing Program Analyst (LPA) D Panlilio conducted an unannounced Health and Safety check due to the department receiving a priority 2 complaint. LPA observed only one client (C1) living at the facility.
During the health and safety check, LPA observed a total of one staff member and one client (C1) at the facility. LPA toured facility with staff, including but not limited to bedrooms, kitchen, bathroom, and common areas. . Licensee (LC) stated C1 just got back from her day program and had a knife hidden inside her backpack. C1 called police, claimed that she had a big knife and wanted to kill herself. When police showed up, LC was not aware that C1 called the police. LC found the knife and locked it.
Paramedics placed C1 on a 51/50 hold and sent her to the psychiatric hospital for treatment.
No deficiencies cited during the health and safety check.
