Facility Evaluation Report

On 12/19/2025 at 12:25pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 11/5/2025. LPA met with Nicole Easton, Program Manager, and explained the purpose of the visit.

This is a follow up case management visit from 11/5/2025. S1 stated the verbal complaint was reported on 11/4/2025 from C1's home. The LPA received the incident report on 11/5/2025 from C1's home. S1 stated S2 and S3 was suspended without pay on 11/5/2025. The facility then conducted an investigation regarding the incident and concluded on 11/6/2025 that S2 would be terminated. The reason for the termination was not abuse but a personal rights violation. S3 was removed from being the 1:1 person for C1 due to not being Phoenix trained.
