Facility Evaluation Report

On 11/5/2025 at 2:55pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 11/4/2025. LPA met with Nicole Easton, Program Manager, and explained the purpose of the visit.

S1 submitted an incident report that occurred on 11/4/2025, for inappropriate behavior from C1 while on an outing at a local park. LPA interviewed three (3) staff during visit. The staff (S3) was assigned to C1. S1 stated there were two (2) staff and two (2) clients present during the incident at the park. S3 stated he was sitting with C2 in the van waiting to go to next park when S4 called S3 to advise C1 was having a behavior because he was denied taking a picture of a little girl. S3 stated C1 and S4 was in the dog park. S3 then contact S1. S1 and three (3) females from C1's home also arrived. S1 stated upon arrival she observed C1 having a bump on his forehead, grass stains on the arm pit of the front of his shirt,
