Facility Evaluation Report

On 11/5/2025 at 11:55am, Licensing Program Analyst (LPA) L. Hall conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 10/29/2025. LPA met with Nicole Easton, Program Manager, and explained the purpose of the visit.

S1 submitted an incident report for an elopement that occurred on 10/28/2025. LPA interviewed three (3) staff during visit. The staff (S3) was assigned to C1. S1 stated there were two (2) staff and two (2) clients present during the incident at the park. S3 stated during interview that C1 kept saying "I want to go that way" as they walked laps at the park. S3 stated on the 4th lap C1 took off through a path. S3 stated he was trained not to chase C1 because that will make him go faster, however to follow at a distance. S1 and the California Highway Patrol (CHP) was contacted because C1 was heading toward the on ramp of the freeway. S1 stated she arrived and was able to get C1 back into the van with a bag of chips.
