Facility Evaluation Report

On 6/3/2025 at 12:55pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Monica Cardoza, Program Director.

While LPA L. Hall was conducting a complaint investigation

15-AS-20250602095755 on 6/3/2025, upon arrival LPA observed a new shed in back of program. LPA toured the back of program and observed there were two (2) sheds built. S1 stated the sheds will be used for crafting by the clients. Facility had not notified Community Care Licensing (CCL).

Deficiency is cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
