Facility Evaluation Report

On 8/28/2025 at 1:15pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced annual required inspection. LPA met with Administrator, Quame King, and LPA explained the purpose of the visit. Administrator certificate #7025965735 expires 8/23/2026.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, garage, common area, and back yard. The facility consists of five (5) bedrooms and two (2) bathrooms. One (1) room occupied by staff. One (1) room used as an office. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 74 degrees Fahrenheit. LPA observed lighting in room is adequate for the comfort and safety of the client. Hot water temperature in the shared clients’ bathroom was measured at 111.4 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for clients. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors/carbon monoxide were in operating condition during visit. Fire extinguisher last serviced on 9/10/2024. Emergency disaster plan last updated 8/28/2025. Fire drill last conducted 8/1/2025. First aid kit was observed to be complete.
