Facility Evaluation Report
On 7/8/2025, at 2:00pm, Licensing Program Analyst (LPA), L. Hall arrived to conduct a case management visit.
On 6/19/2025, LPA received an email stating that Administrator/Licensee no longer has control of the property as of May 31, 2025. Administrator/Licensee also submitted a copy of the license. LPA spoke with Administrator/Licensee on 6/25/2025 and was advised the license was never received via USPS mail. Licensee/Administrator gave forwarding address to LPA.
LPA arrived at the facility today and observed a silver Toyota Sienna parked in front of the house, a white Toyota Rav4 and a white Toyota Tacoma in the drive. There was also 4 pairs of shoes on the porch and an EV Charger was installed on the house. LPA knocked on the door and rang doorbell but did not receive a response.
No one to conduct the exit interview with or provide a copy of this report.
