Facility Evaluation Report

On 1/14/2026 at 3:00 PM, Licensing Program Analyst (LPA) A. Gomez arrived announced to conduct 1-Year Annual Required inspection. LPA met with House Manager, Naomi Loquellano Richards and explained the purpose of the visit. The facility is temporarily closed due to a fire that occurred in April of 2024.

LPA observed that the facility is still under construction. The facility does not currently have running water or electricity. The facility is currently undergoing construction. There are no staff or residents. Facility is projected to be done with construction by February 2026 however House Manager states that they have decided not to reopen and plans on forfeiting their licence. LPA advised Licensee to submit their notice of closure

No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
