The inspector’s account

On 8/29/2025, at 1:15pm Licensing Program Analyst (LPA) Noel Wolf Petersen arrived unannounced to conduct an annual inspection, the LPA met with administrator Shawna Swing-Rudisill to explain the purpose of the visit. Karen Cares is a 4 bed facility serving adults aged 18-59. There are currently 4 residents in care, none have restricted conditions. There are no exceptions or waivers present for the facility.

The Physical Plant was inspected, including but not limited to the kitchen, client bedrooms, client bathrooms, the exterior, the storage areas, and the evacuation routes. The facility is clean, traffic areas are well lit and unobstructed. The required postings are present and of the correct dimension. The kitchen has adquate storage for sharps, medicaion, and toxics. The bedrooms have required furniture and furnishings. The bathrooms have functioning hardware and water temperature is delivered to the standard of 105-120*F. Storage areas are used for resident goods. The exteriors are clean, no tripping hazards, and have plenty of space for activites. The evacuation routes are in good repair.

LPA reviewed 4 of 4 Client files, documents were found to be present and up to date including but not limited to: signed admission agreements, medical/mental health preapprasials, Ipps/needs and services plans, bonded cash and property.
