Facility Evaluation Report
On July 29 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct an unannounced case management visit in response to an incident Report received on 05-06-26. LPA met with care staff MC. LPA Benson observed one care staff and four residents. Care staff MC phoned Administrator Brittany Morton and LPA Benson explained the purpose of the visit. The administrator arrived ten minutes latter.
LPA Benson toured the facility with care staff.
LPA Benson reviewed the Incident report, interviewed the Administrator and performed a medication file review. Interview is attached in an 812.
An exit interview was conducted, and a copy of the report was left with the administrator.
No deficiencies are being cited as a result of today’s visit.
