Facility Evaluation Report

On 07/01/2026 licensing program analyst Marisa Chiarelli arrived at the facility announced for a case management visit. LPA Chiarelli met with Anthony Faulkner and explained the purpose of the visit.

Following an annual inspection conducted on 06/11/2026 LPA Chiarelli was informed that Anthony Faulkner wanted to become the new administrator for Sunshine assisted living - The House . Anthony would be replacing the previous administrator Maria Boulangger Falla who is no longer working at the facility since April 2026. It was determined that Anthony's administrator certificate is expired. Resident Care Coordinator Dayna Mock is getting her administrator certificate and possibly will be taking over as administrator for The House.

One deficiency is being cited as of today's visit.

Exit interview conducted copy of report and appeal rights given to the administrator.
