Facility Evaluation Report
05/14/2025 12:45 PM Licensing Program Analyst Rebecca Knight conducted an unannounced case management visit and met with licensee / administrator Annette Frazier. The purpose of the visit was to issue a citation related to a non-compliance conference that was held in the Chico office on April 9, 2025. Licensee was not available for the visit but agreed to conduct the visit over the phone and LPA read the report to licensee. Licensee agreed to sign the report and email back to LPA.
During the non-compliance conference the licensee agreed to hire an administrative assistant who had obtained current DOJ background clearance by May 7, 2025. On May 7, 2025 LPA determined that the licensee had not hired an administrative assistant. Licensee requested a one week extension on the due date of this requirement, LPA granted this extension with a new date of May 14, 2025. On May 14, 2025 licensee informed LPA they had not hired the administrative assistant and requested another extension of the due date to May 30, 2025. LPA is issuing a citation as a result. Failure to correct the deficiency by the due date will result in civil penalties being assessed.
A deficiency is being issued as a result. California Code of Regulations, (Title 22), is being cited on the attached LIC9099D. Appeal rights were provided. Exit interview conducted and a copy of the report was provided to licensee Annette Frazier.
