Facility Evaluation Report

09/15/2025 2:00 PM An informal conference was conducted today via TEAMS. The purpose of this informal conference meeting was to discuss the progress the facility has made since hiring an administrative assistant and to obtain general information about the facility. Present in the meeting today were Licensing Program Manager, Lauren Crocker, Licensing Program Analyst Rebecca Knight, Far Northern Regional Center Case Management Supervisor Michelle Whetmore, Stepping Stones licensee Annette Frazier, and Stepping Stones administrative assistant Elizabeth DeFronzo.

Issues discussed during the meeting were:

· Status of facility’s Title 22 requirements for their files.

· Requirements to obtain and track staff training and CEU’s.

· Plan of correction requirements related to the facility’s 2025 annual inspection.

The facility has stated they will do the following to achieve continued and substantial compliance:

· Complete all plan of correction requirements related to 2025 annual inspection by the due date of 10/03/2025. Facility agrees to send LPA photographs as the requirements are completed. LPA will follow up with a visit to the facility after the POC due date.

· Ensure that a process is in place for completing and submitting reports to Far Northern Regional Center.

No deficiencies were cited as a result of the meeting.
