The inspector’s account

04/17/2025 10:00 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with administrator Nora Serrano. The purpose of this visit was to conduct a case management investigation.

It was reported that on 04/08/2025 Client 1 (C1) was arrested for not completing their annual 290 registration.

LPA reviewed C1’s IPP and admission agreement. IPP states that C1 is required to register their 290 status annually.

It was learned that it is the facility’s responsibility to ensure C1 completes their annual 290 registration and overlooked the due date by mistake.

Based on interviews, documents and evidence obtained during the investigation, the preponderance of evidence standard has been met, therefore, the allegation that facility failed to ensure that a client completed their required annual 290 registration is found to be SUBSTANTIATED. California Code of Regulations, (Title 22), is being cited on the attached LIC9099D. Appeal rights were provided. Exit interview was conducted and the report was provided to administrator Nora Serrano.
