Facility Evaluation Report
11/25/2025 11:20 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with Executive Director April Kale. The purpose of this visit was to obtain current status on the items that LPA issued a physical plant citation for on 08/28/2025. This is LPA’s third follow-up visit to ensure compliance.
LPA and ED toured the facility together and LPA made the following observations:
The facility wifi / internet had not been functioning since 08/11/2025.
During today's visit LPA learned that the new internet provider completed the installation of the new services on 10/30/2025. This deficiency has been completed.
The carpet in the memory care unit and assisted living common areas is stained and dirty.
LPA observed the carpets in the memory care unit and assisted living common areas to still have some minor staining but the cleanliness of the carpet has been improved. ED confirmed that the carpets have been cleaned six times since 08/28/2025. The carpets are being professionally cleaned every two weeks. ED has been sending LPA the receipts for all carpet cleanings and will continue to send LPA receipts for all carpet cleanings moving forward. The licensee has requested bids for the replacement of all carpet in the common areas of the memory care and assisted living sections of the building and will send LPA the winning bid once the contract has been agreed to. LPA will continue to monitor.
The fence that surrounds the garden in the memory care unit is in disrepair.
LPA observed that the facility has replaced the entire fence and gate in the garden area. The licensee has removed a large pine tree that had fallen some time ago. This deficiency has been completed.
No deficiencies were issued as a result of today's visit. Exit interview was conducted and a copy of the report was provided to Administrator April Kale.
