01/08/2026 11:30 AM Licensing Program Analyst (LPA) Rebecca Knight conducted an announced visit to the facility and met with Licensee Nora Serrano.

All clients move out of the facility on 09/01/2025 and the home has been empty since that date. There were no clients present in the facility during today's visit.

Licensee agreed to shred the facility license.

No deficiencies were cited during this visit. The report was provided to licensee Nora Serrano.
