Facility Evaluation Report

09/022025 02:00 PM Licensing Program Analyst (LPA) Rebecca Knight conducted an announced visit to the facility and met with Licensee Nora Chavez. The purpose of the visit is to discuss options for the facility.

There are currently no clients in care in the facility. Licensee plans to keep the facility license until the lease ends in December 2025 even though there will be no clients in care for the remainder of her lease. Before the lease ends licensee agrees to notify LPA of their plan to either close the facility and surrender the license or transfer the license to a new location for another adult residential facility.

No deficiencies were cited during this visit. A copy of the report was provided to licensee.
