The inspector’s account

11/04/2025 11:07 AM Licensing Program Analyst (LPA) Rebecca Knight, made an unannounced visit to the facility and met with licensee/administrator Brian Moebs. The purpose of this visit was to conduct a case management investigation regarding two occurrences of the same client eloping from the facility.

It was reported that on 10/09/2025 08:30 PM the administrator received a text from Client 1 (C1) family member informing them that C1 would be staying with the family member for the night. The administrator called house staff to inform them of C1's plans. Staff thought C1 was still at the facility in their room. Staff knocked on C1's door and entered the room when they did not receive a response. Staff discovered that C1 was not in their room and observed that the window screen had been removed and was leaning against the wall inside the room. Administrator reviewed video footage of the exterior facility cameras and observed C1 climbing out of their window at 08:15 PM the same evening. On 10/10/2025 09:15 AM C1 returned to the facility.

A second incident report regarding C1 was received on 10/23/2025 regarding an incident that occurred on 10/22/2025. While reviewing exterior facility video camera footage administrator reports that morning at 02:37 AM C1 removed their bedroom window screen, a flash of light from a lighter was observed and C1 stuck their head out of the window blowing smoke outside. Then C1 climbed out of their window and walked down the street. At 04:16 AM C1 arrived back at the facility and climbed back in their bedroom window, replacing the window screen. When administrator questioned C1, C1 claimed to have no recollection of these events.

In order to prevent this from happening again the licensee has been counseling C1 and has confirmed that C1 understands the house rules which state that smoking is not allowed within the facility. C1 is able to leave the facility unassisted and understands they do not have to climb out of their bedroom window but can leave through the front door and inform staff. C1 has not had any additional incidents since and has expressed that they would like to remain living in the facility.

No deficiencies were cited as a result of the visit.
