Facility Evaluation Report
07/23/2026 12:40 PM Licensing Program Analyst Rebecca Knight conducted an unannounced case management visit and met with administrator Nicole Heap. The purpose of the visit was to obtain documents for three (3) clients who attend the day program.
LPA requested the following documents: Admission agreement, care notes, observation notes, for three (3) clients, outing schedule, activity calendar, attendance sheets, staff schedule for June and July 2026, client list, staff list with telephone numbers.
No deficiencies were cited as a result of today’s visit. Exit interview conducted and a copy of the report was provided to administrator Nicole Heap.
