The inspector’s account

On December 11, 2025, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to conduct a Required Annual Inspection. LPA met with Program Coordinator, Amy Brown, and explained the purpose of the visit. At the time of the inspection, 22 clients and 8 staff were present in the facility. LPA observed clients to be participating in an exercise class, learning letter/word recognition, and working on motor skills development. .

LPA Adkison and Program Coordinator toured the facility together to ensure the health and safety of clients in care. Areas toured include but are not limited to: common areas, a sensory room, therapy room, one (1) bathroom, kitchen, changing rooms, and storage areas. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA observed all walkways and ramps to be free of clutter or obstructions.

All areas observed were found to be clean and in good repair. All clients bring their lunches from home and staff assist with preparation in a microwave if needed. All cleaning supplies, sharps, and medications were observed to be kept locked and inaccessible to residents to care. LPA observed the facility Medication Administration Records (MARs) which were found to be accurate. There is a schedule of activities and classes posted in the main classroom for clients to observe.

The facility was observed to be at a comfortable temperature. Fire extinguishers are due to be inspected in January 2025. There were no carbon monoxide detectors in the facility. The facility maintenance supervisor purchased and installed three (3) detectors during the inspection. Water temperature was recorded within Title 22 regulations. The last disaster drill was conducted and documented in October 2025. The facility has been conducting drills monthly.

LPA reviewed five (5) staff files and (5) client files which contained all the required documentation. All employees requiring background checks are cleared. All first aid and CPR training was current.

One (1) deficiency is being cited as a result of this inspection. California Code of Regulations, (Title 22), is cited on the attached LIC 9099-D. Exit Interview conducted. A copy of this report and Appeal Rights were provided to Amy Brown, Program Coordinator, via email.
