Facility Evaluation Report
On February 18, 2026, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to conduct a Required Annual Inspection. LPA met with Jenni Richmond, Program Coordinator, and explained the purpose of the visit. At the time of the inspection, two (2) clients and three (3) staff were present in the facility. LPA observed clients utilizing exercise equipment and listening to music during the inspection.
LPA Adkison and Program Coordinator toured the facility together to ensure the health and safety of clients in care. Areas toured include but are not limited to: activity rooms, storage areas, kitchen, office and one (1) bathroom. The facility has a plethora of activities for client use including exercise equipment, games, puzzles, art, and computers. The facility utilizes primarily individualized programs for clients, however, also offers music and cooking classes as group activities.
All areas observed were found to be clean and in good repair. LPA observed all walkways to be free of clutter or obstructions. Most clients bring their lunches from home. There is additional food stock for staff to assist clients with preparing their own lunches. LPA observed all sharps, cleaning supplies, and other toxins to be locked away and inaccessible to clients in care. Hot water temperature measured within Title 22 regulations. LPA reviewed the Medication Administration Record (MAR) for one (1) client and found it to be in compliance.
The facility was observed to be at a comfortable temperature. Fire extinguishers were fully charged and last inspected in June 2025. Smoke and carbon monoxide detectors were observed and were in working order. The last disaster drill was conducted and documented in June 2025. LPA and Program Coordinator discussed conducting emergency drills quarterly to remain in compliance with Title 22 regulations.
LPA reviewed three (3) staff files and (3) client files. All employees requiring background checks are cleared. All CPR/First Aid training was current. One (1) of three (3) staff were not associated to the facility, however, LPA observed a Fingerprint Clearance and a Criminal Record Exemption Transfer Request dated June 2025 in the staff member's file. LPA forwarded the information to Community Care Licensing (CCL) support staff to have the staff member appropriately associated.
