The inspector’s account

05/08/2025 09:30 AM Licensing Program Analyst (LPA) Rebecca Knight arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with administrators Brian Perry (7000552740 exp. 06/02/2025) and Cindy Denny (700417174 exp. 03/25/2026) and explained the purpose of the visit.

LPA Knight and the administrator toured the facility together to ensure health and safety of residents in care. LPA observed the facility to be clean, in good repair and odor-free. In the areas toured no immediate health, safety, or personal rights violations were observed. Medication is locked in a secured med room and a locked med cart.

Areas toured include but are not limited to: common areas, kitchen, resident bedrooms, common restrooms, and garden areas. LPA observed each bathroom and shower room to have the necessary grab bars, non-skid flooring or shower chair. LPA observed each resident bedroom to have the required furnishings, working lights and windows with screens. LPA observed activity calendar located in the common area. LPA observed the kitchen to be clean, food appears to be prepared and stored properly. Facility has a 2-day perishable and a 7-day non-perishable amount of food.

LPA observed a very nice garden area with multiple shaded structures with ample seating for the residents to enjoy. LPA observed fire extinguishers, fire detectors, and carbon monoxide detectors. LPA observed the chemicals and medications to be locked and inaccessible to residents. The facility has been conducting quarterly emergency evacuation drills and quarterly fire drills.

LPA reviewed a total of five (5) residents files and four (4) staff files which contained all of the required documentation. All staff have required background clearance on file. LPA requested current administrator certificates to be kept on file in the Chico office.

No deficiencies are being cited as a result of today’s inspection. Exit interview conducted and copy of report was provided to the administrator.
