Facility Evaluation Report
On April 8, 2026, Licensing Program Analyst (LPA) Kayla Adkison and Licensing Program Manager (LPM) Lauren Crocker arrived at the facility unannounced to conduct a Required-1 Year Inspection.
The program is located inside of licensed facility Roseleaf Gardens (Lic # 45002775 ). Although licensed, the day program has not been operating since 2020 and was not in operation at the time of this inspection.
Per administrator Grace Hawkins, (Roseleaf Gardens Lic # 45002775) there is no current intention to reopen the program.
No deficiencies are being cited as a result of todays inspection. Exit interview conducted and copy of report was provided to Administrator, via email.
