Facility Evaluation Report

On April 8, 2026, Licensing Program Analyst (LPA Kayla Adkison and Licensing Program Manager (LPM) Lauren Crocker arrived at the facility unannounced to conduct a Required-1 Year inspection. This facility has not been in operation and does not have an active administrator or any residents in care.

LPA toured the exterior of the facility to ensure that it is not in operation and confirmed there are no residents living on the property.

T his facility is currently in the process of being sold and will not be reopened as a Community Care licensed facility.

No deficiencies being cited as a result of todays visit. Exit interview conducted and a copy of this report is left at the facility
