Complaint Investigation Report
The Department has investigated the allegation and found the allegation to be unsubstantiated. The Department reviewed relevant documentation to include medical and conducted interviews with residents, and facility staff. The information obtained did not support the allegation. Staff were unaware the resident had sustained an injury nor did they observe an injury prior to sending the resident to the hospital for an unrelated medical condition.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
