Facility Evaluation Report
On October 9, 2025, at approximately 3:30 PM, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to follow up on a recent incident report submitted by the facility. LPA Adkison met with Administrator, Maxine Mabel, and explained the purpose of the visit.
On September 25, 2025, and September 26, 2025, the facility faxed (2) two incident reports to Community Care Licensing (CCL) documenting a resident's fall at the facility where the resident sustained a broken femur that determined to require immediate surgery. On September 27, 2025, the surgery was conducted at Enloe Hospital in Chico.
On October 6, 2025, the facility submitted an additional incident report documenting the client's death while recovering in the hospital.
During the visit to the facility, LPA and Administrator toured the facility and made observations. LPA collected several supporting documents and conducted (1) one interview. Additional documents were requested to be emailed to the LPA once they are available.
Exit Interview conducted. No deficiencies are being cited at this time. At. A copy of this report was provided to Administrator, Maxine Mabel, via email. dministrator needed to leave to assist other clients with doctor's appointments and verbally approved staff signing off on this repor
