Facility Evaluation Report
On August 1, 2025 Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to confirm the facilities closure. No one was present at LPA's arrival.
On June 13, 2025, LPA received notice from the Administrator of the intent to close the facility effective July 31, 2025. The Administrator reported the property is being sold as a residential home and will no longer serve Regional Center clients. The facility further reported that the facility has secured alternative housing for all clients in care to ensure a smooth transition prior to the closing of escrow on the property.
LPA rang the door bell with no reply. LPA observed no residents in care.
No deficiencies were cited during this visit. A copy of the report was emailed to the licensee for a signature.
Licensee will return a copy of signed report to licensing.
