Facility Evaluation Report
On June 17, 2026, Licensing Program Analyst (LPA) Kayla Adkison arrived at the facility unannounced to conduct the required annual inspection. LPA met with Nedina Tuck, Facility Manager, and explained the purpose of the visit. During the inspection, there were 8 clients and 6 staff providing direct care and supervision. LPA observed clients resting in their rooms, watching television, and participating in a budgeting group. LPA and Facility Manager toured the facility together to ensure the health and safety of clients in care.
Areas toured included but are not limited to: common areas, resident bedrooms and bathrooms, backyard, sand storage areas. All walkways and ramps were free of hazards or obstructions. LPA observed resident bedrooms to have the required furnishings. Facility has a 2-day perishable and a 7-day non-perishable amount of food. LPA observed all sharps and medications to be locked and inaccessible to residents. There is a schedule of activities and programs posted for residents.
In the areas toured no immediate health, safety, or personal rights violations were observed. The facility is conducting emergency disaster drills with the last drill being documented in June 2026. LPA observed a completed emergency first aid kit ready fully stocked and ready for use. LPA observed fire extinguishers to be fully charged and last inspected in February 2026.
LPA reviewed three (3) client files which contained all required documentation. LPA reviewed three (3) staff files, one of which was missing a current CPR/1st Aid certification.
Deficiencies are being cited in compliance with Title 22 of California Code of Regulations (see attached LIC 809-D). Exit interview conducted. A copy of this report, and Appeal Rights, were provided to Administrator, via email.
