Facility Evaluation Report
On September 3, 2025, Licensing Program Analyst (LPA) Kayla Adkison and Licensing Program Manager (LPM) Lauren Crocker arrived at the facility unannounced for the purpose of a case management visit. LPA and LPM were joined by Far Northern Regional Center (FRNC) Resource Development Quality Assurance Specialist, Cecina Hines. LPA and LPM were greeted at the facility by Romelina Cook, Licensee. During the visit, a tour was conducted and observations were made. No residents were present at the facility during the visit.
The purpose of the meeting was to conduct a follow-up after the licensing failed to attend a previous scheduled Microsoft Teams Meeting to discuss some concerns. During the visit, several topics were discussed including, but not limited to, the potential sale of the property and the submission of a closure plan to Community Care Licensing Division (CCLD) and Far Northern Regional Center (FRNC). Additionally, the Licensee was directed to have the language in the "For Sale" advertisement of the property changed on Zillow.com immediately. During the visit, Licensee facilitated a telephone conversation with LPM and the real estate agent to discuss further requirements.
The licensee was directed to compose a letter for clients in care explaining the circumstances and a timeline of the sale. CCLD encouraged as much time as possible to be noted in the letter addressed to clients. The licensee was further directed to compose a letter addressed to CCLD and FRNC explaining the same information.
Deficiencies are being cited on the attached LIC 809-D pursuant to Title 22 regulations. Exit interview conducted and a copy of this report, and appeal rights, were provided to Licensee/Administrator, Romelina Cook, administrator, via email.
