Facility Evaluation Report
On July 22 2026 Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct a case management concerning staff with criminal background clearances. LPA Benson met with the Administrator Kelly Doty. LPA was informed Administrator Lornal Pham is no longer here. LPA requested required papers for change in Administrator.
LPA toured the facility and reviewed staff rosters to confirm all staff have criminal background clearances. LPA Benson shared information concerning licensing site and the CARE tool.
(LPA) Sarah Benson conducted an audit of criminal record clearance of facility roster on Guardian and discovered S1 has been associated to the facility. S1 has not been cleared by criminal background.
When asked how long has S1 been working at the facility, Staff reported S1 does not work at this building.
Exit interview conducted and a copy of the report was provided to the administrator.
