An unannounced collateral visit made out to the facility on 5/15/2025 at 1PM, by Licensing Program Analyst (LPA) Arvin Villanueva who was met by Administrator Karyn Gregorious .
LPA explained the purpose of this collateral visit was to interview client in regards to a compliant control # 27-AS-20250508123135 that is unrelated to this facility. During today's visit LPA interviewed 2 clients (C1 and C2) who attend this day program.
Per California Code of Regulations, Title 22, no deficiencies were observed during this visit. Exit interview was conducted and a copy of this report was provided.
