Facility Evaluation Report
On 7/30/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA met with Administrators Carol and William Schwabe, and stated the purpose of the visit. Present during this visit were 2 staff. Clients in care were out in the community during this visit.
LPA inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards. LPA observed sufficient furniture and lighting throughout the facility. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Hot water temperature was measured at 106 degrees Fahrenheit in 1 of 2 resident bathroom sink. Room temperature was at 72 degrees Fahrenheit.
Fire extinguishers and smoke detectors are current and in compliance with fire safety. LPA observed centrally stored medications locked in each section of the facility. LPA reviewed 1 of 2 resident files. LPA reviewed 2 staff files, including criminal record clearances. All staff are fingerprint cleared and associated to the facility. First aid kit was checked and is complete. Fire drill was completed on 6/7/25.
LPA requested a copy of the following documents: LIC500 (Personnel Roster), LIC308 (Designation of Responsibility) , Surety Bond certificate.
No deficiencies were cited. Exit interview conducted and copy of the report was provided.
