Facility Evaluation Report

On 7/10/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA met with Administrator Zully Six (S1), and Co-Administrator Mechelle Silva (S2) and stated the purpose of the visit.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents, ages 18 to 59, all ambulatory only. Facility is also vendored by Valley Mountain Regional Center. Facility has 4 resident bedrooms and two resident bathrooms, and one staff room. Laundry room has an exit to the outside. One resident bedroom has an exit to the patio.

Initial Observation: Upon arrival, one resident was present, in their bedroom. One staff on duty (S3) was also present along with S1 and S2. LPA observed required posters and facility license at the entrance. Room temperature was at 76 degrees Fahrenheit upon arrival.

Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA observed the inside of the facility to be clean and in good repair at this time. LPA inspected 3 of 4 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. LPA measured the hot water temperature in 1 of 2 bathrooms; it was measured at 112 degrees Fahrenheit. Both resident bathrooms were observed to be in clean and good repair at this time. LPA observed the garage to be downstairs where they keep additional food supply. The garage is not considered an emergency exit, according to staff.
