Facility Evaluation Report
On 8/27/2025, at 9:30am, Licensing Program Analyst Arvin Villanueva (LPA) arrived at facility unannounced to conduct an inspection proceeding the closure of the facility. LPA met with Facility Supervisor Renae Earl (S1) and explained the purpose of the visit. A Notice of Facility Closure was received by the Regional Office with a noted facility closure date of 9/18/2025 .
LPA toured the facility with a staff Lacreisha Wilson (S2). LPA observed that there were no residents at the facility.
LPA requested for Licensee to update the resident roster.
LPA requested original License from the Licensee.
LPA advised Licensee that the facility will be closed in the system as of 9/18/2025.
