Facility Evaluation Report

On 7/30/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived announced at this facility to conduct the annual inspection visit. LPA met with a staff and stated the purpose of the visit. The Administrator, Aaron Cocjin, was notified of this visit and arrived shortly after, but needed to leave immediately due to an appointment and gave permission to staff Genardino to sign this report.

Currently, there are no residents living in the facility. Administrator Cocjin confirmed that they want to keep the license active and that they currently have inactive vendor status with Valley Mountain Regional Center (VMRC). The facility is licensed to serve up to 4 adult residents. Rooms 1 and 2 are approved for non-ambulatory use and Room #3 is approved for 2 ambulatory residents only.

During the visit, the LPA and staff did a physical inspection of the facility, which included inspecting the kitchen, living area, resident bedrooms, hallway bathroom, and outdoor spaces. The room temperature was set at 74°F. The hot water temperature in the hallway bathroom was measured at 106°F. The LPA provided advice to fix the curtain rod in bedroom #2 and repair the exit gate, which, per staff, was broken by a gardener. The LPA also advised fixing the door of the storage shed if the licensee plans to store any dangerous items once they have residents in care.

Since there are no residents and facility is not currently inactive, LPA did not use the CARE TOOLS. There were no residents to interview or relevant records to review at this time. Advisory was provided to stay current with licensing fees.

Based on today's visit, no deficiencies are being cited. Exit interview was conducted and a copy of this report was provided.
