The inspector’s account

On 3/27/2025, Licensing Program Analyst (LPA) Arvin Villanueva arrived unannounced at this facility to conduct a case management - annual continuation to continue the annual inspection initiated on 3/26/2025. LPA was met with staff on duty, Jodelyn Delos Santos (S1) and stated the purpose of this visit. The home manager, Aaron Cocjin, was notified of this visit and arrived at 11:45am. Present during this visit were 1 clients in care with 1 staff on duty. The other residents in care were out in the community.

The LPA continued with facility visit to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. During this visit, LPA reviewed facility's infection control plan. Advisory was provided to review their infection control plan annually or as needed to assess any necessary changes if needed. Facility conducts monthly fire drills and last drill was conducted on 3/6/2025 at 8:30am. Facility also conducts disaster drill at least twice a year. Last disaster drill training was conducted on 12/18/2024.

LPA reviewed three (3) resident files and (3) staff files, including criminal record clearances. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. LPA reviewed P&I money for 2 residents with S1 and were found accurate. LPA reviewed medications for 2 residents.

LPA requested the following documentation: LIC 500, LIC 308, updated Facility Sketch, Surety Bond, and current liability insurance certificate.

Per California Code of Regulations, Title 22 Division 6, Chapter 6, deficiencies were cited during this visit.

Exit interview was conducted with Aaron Cocjin and a copy of this report and appeal rights were provided.
