On 7/22/2026 at 1:30 pm, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a case management visit to deliver amended report originally dated 7/16/2026. LPA met with Ferdinand Gutierrez, Administrator and informed him the reason for visit.
LPA obtained the original report dated 7/16/2026 from the Administrator.
No deficiencies are being cited on this date.
