Facility Evaluation Report

Licensing Program Analysts (LPA) Valeria Conway conducted an unannounced Case Management - Other visit to the facility above. LPA contacted the Licensee, Jocelyn Cacal via telephone and explained the reason for the visit. The purpose of this visit is to check on the status of the relocation of the residents, to ensure that no additional people are residing at this location and to conduct a closure visit, as this location given a Change of Location (CHOL) was approved at 4104 Clubhouse Dr. Somis, CA. 93066 with new facility # 565850614.

During today’s visit, LPA arrived at the facility at 10:40 A.M. and rang the bell but no one answered. LPA observed a locked gate, there were no cars parked either outside or inside the location. LPA contacted the Licensee via phone at 10:55 A.M. The Licensee agreed to meet with LPA to conduct a final walk-through of the facility prior to closure. At 11:25 A.M. Licensee arrived at the facility and granted access to the LPA. LPA observed no one present. Per the licensee, all residents were relocated to the new location on 08/15/2025. LPA did not observe any signs of care and supervision being provided at the facility.

The Licensee was unable to surrender the license during today’s visit. Per the Licensee the physical copy of the license was misplaced. As of 08/26/2025, facility # 565850272, Somis Living II, located at 4115 Sand Canyon RD. Somis CA. 93066 is closed.
