Licensing Program Analyst (LPA) Zabel Chochian conducted an unannounced Case Management
Deficiency visit in conjunction to complaint visit (CC#29-AS-20231013151652).
LPA met with Ernest Federer, Administrator. The purpose of this visit is to issue citations for deficiencies observed during the course of the complaint investigation which was not related to the complaint (#29-AS-20231013151652).
On 10/19/2023, at approximately 10:45am five clients’ medication records and medications were reviewed with the medtech staff. Following discrepancies were observed: Client #2’s medication record indicated that there were 9 (pregabalin) pills; actual count of pills remaining in the bottle was 17; another medication (risperidone) was recorded as 18 pills remaining in the bottle, however the was 9 pills left in the bottle.
On 12/19/2023, a subsequent complaint visit was conducted and during the course of investigation it was revealed that Simi Oaks Day Program staff and ED admit to giving Client #1 (C1) tic-tacs in lieu of medication as a strategy to mitigate C1’s behaviors.
Citation issued, exit interview held, appeal rights given.
