Facility Evaluation Report
Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct a required annual visit today. Upon arrival, the LPA met with the Administrator, Maria Castillo, and at this time explained the reason for the visit. There is one (1) resident at this facility; however, they were at day program at the time of the inspection. Entrance interview conducted.
Beginning at 12:15 p.m., the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that facility is in compliance with Title 22 Regulations. The following was observed:
Kitchen: The LPA observed the kitchen/food area at approximately 12:25 p.m. The kitchen appliances appeared to be clean and in operable condition. The facility has a sufficient supply of non-perishable and perishable food; properly stored. Refrigerator and dry food pantry were checked for proper labels and expiration dates.
Bedrooms: There is one (1) bedroom designated for resident use. The LPA observed bedroom to be properly furnished with a bed, furniture chest with drawers, a chair, and sufficient lighting. Staff room was observed on premises.
