The inspector’s account

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit for the purpose of investigating a self reported incident report. Upon arrival, LPA met with Program Manager Jessi Chevez and explained the reason for the visit.

On 06/16/2026, at approximately 10:00 a.m., Staff #1 (S1) reported the following incident to Program Director Jessi Chevez: S1 and Staff #2 (S2) were assisting Client #1 (C1) in the restroom. While S2 was attempting to fasten the seatbelt on C1's wheelchair, C1 attempted to strike S2 in the face with their knee. C1 did not make contact with S2. In response, S2 struck C1 twice on the left knee with a closed fist and then forcefully pushed C1's leg down onto the wheelchair. Following the incident, C1 was immediately assessed by the facility nurse. No bruising or other visible injuries were observed, and C1 did not display signs of pain or discomfort. During C1's next visit to the facility, C1's left knee was reassessed, and no bruising or other visible injuries were observed. S2 was placed on administrative leave pending the outcome of the facility's internal investigation.

At approximately 10:00 a.m., the Licensing Program Analyst (LPA) conducted a physical plant inspection, interviewed staff, and reviewed and obtained copies of records relevant to the investigation. During the visit Program Director Jessi Chevez spoke with their Quality Improvement Specialist who stated their internal investigation will be completed by the end of the week. Further investigation required at this time.
