The inspector’s account

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at approx 10:10 a.m. for the purpose of investigating a self-reported incident report. LPA was joined by Tri-Counties Regional Center (TCRC) Quality Assurance Specialist (QAS) Ryan Landseadel. Upon arrival, LPA and QAS met with Area Director Leah Kolbeck and explained the reason for the visit.

On 7/25/2025 at the start of her 4pm shift, Cole Vocational Services (CVS) Simi Valley, Staff #1 (S1) was reviewing medications and noticed that Client #1(C1) did not receive their 11am prescribed medication Metoclopramide HCL 10mg tab given via g-tube.

At approx 10:00a.m. LPA and QAS conducted physical plant, interviewed staff, as well as reviewed and obtained copies of pertinent documentation relevant to the investigation.

Pursuant to Title 22 of the California Code of Regulations Division 6, Chapter 8, the following deficiencies were cited (refer to LIC 809-D). Failure to correct the deficiencies may result in additional civil penalties.
